Free COVID-19 Vaccination Clinic

Golden Valley High School

10 a.m. - 4 p.m.

Participants will receive their first dose of the Pfizer vaccine

Families and students 12-and-over are welcome to sign up

On the day of clinic, please bring:

Proof of Age (i.e. photo ID, school transcript, etc) (You do not need to show government issued ID and you do not need to be a US citizen to get a vaccine.)
Copy of insurance card (COVID-19 vaccines are free regardless of insurance status)
Completed consent/release form (parent/guardian's signature is required for students under 18). See the form attached to this email
An Adult MUST accompany a minor aged 12-15

Second dose appointment will be automatically booked for October 8, 2021 at Golden Valley HS
